Phytel Launches
Atmosphere Platform for Care Management Automation
September 21, 2011
Phytel’s
Atmosphere platform allows care teams to deliver appropriate care
efficiently across their entire population, regardless of care setting.
Atmosphere uniquely combines automated interventions that ensure
patients receive the care they need with analytic reports on the
effectiveness of quality improvement programs. The open-architected
Atmosphere™ platform also offers companies that partner with Phytel
partners the ability to securely integrate consumer health, education,
digital coaching and other applications relevant to population health
management.
Among the core elements of the care management suite running on Phytel
Atmosphere are:
- protocol-driven patient-centered
registries that automatically identify care gaps and trigger
messages to patients for recommended care;
- applications that stratify patient
populations according to identified health risks and create
personalized, automated interventions;
- patient self-management programs that
include health risk assessments and individualized online care plans
- automated provider-branded communications
that follow a patient's hospital or ER discharges to help prevent
unnecessary readmissions;
- sophisticated analytics populating
dashboards that measures an organization's effectiveness in its
quality improvement initiatives;
- automation tools that identify patient
groups and processes required to qualify for Medicare's PQRS rewards
and streamline the application for those incentives;
- automated communication that proactively
reminds patients about upcoming appointments with their providers.
Hundreds of leading
physician groups and healthcare systems have acquired components of
Phytel's innovative suite of care management solutions, including
Allegiance Health, Bon Secours Health System, MedVirginia, The Iowa
Clinic, Riverside Medical Group, Christie Clinic, Elmhurst Clinic and
Prevea Health. All of these organizations are committed to clinical
transformations that encompass population health management and the
development of medical homes and/or accountable care organizations.
"We see Phytel as an integral piece of our effort to build the
patient-centered medical home," said Kimberly Freese-Beal, MD of
Allegiance Health. "Phytel helps us do that with lean resources, through
automation, and by delivering information to providers and staff. Its
solutions enable us to reach patients who are not getting recommended
care. As we tailor our care to better meet our patients' needs, Phytel's
risk stratification capability will allow our care managers to quickly
identify the patients who need more intensive care management. And the
quality dashboards that show variations in practice and allow our
doctors to more clearly converge toward evidence-based medicine will be
very helpful from a quality perspective."
“Phytel’s
solutions provide a great benefit to those who are fully committed to
population health and the patient-centered medical home,” said Ashok Rai,
MD, president and CEO of Prevea Health. "Phytel gives physicians the
right automated tools at the right time to engage patients and manage
those who are not only in front of them, but also those who need to be
seen. There aren’t enough hours in the day for physicians to do all of
what they are being called on to do. Phytel is offering an automated
solution to make data actionable for better outcomes across a patient
population while allowing physicians and care teams to focus on what
they do best.”
Steve Schelhammer, CEO of Phytel, commented, "Our newest suite of care
management solutions is designed to help providers deliver optimal care
to their entire patient populations. By supplying care teams with the
necessary automation tools, identifying and contacting patients with
care gaps, and supporting patients' engagement with their providers,
Phytel gives providers the capabilities to meet their patients’ entire
preventive and chronic care needs while reinforcing our clients’
financial performance. This next generation of care management
technology is a game changer and stands in stark contrast to the largely
manual and disconnected approaches that have traditionally been used in
this field. We are leveraging technology and data visibility to deliver
powerful automated benefits that are crucial for success in the emerging
population health management model of healthcare delivery." |
